Recently I broke the radius head of my elbow and my surgeon believes I may have torn most, if not all 3 ligaments in my elbow. Question: what do I need for a disability permit to hunt with a cross bow? I currently have a "lifetime" license.

You need to fill out the application form and have your doctor fill out the doctor's form. Then send them both in. It usually take about 3 weeks.
